Puppy Application Form

Thank you for taking the time to fill out this questionnaire to help us in finding the right puppy for you. Please feel free to call or e-mail us if you wish more information or need questions answered. If you would like to come and visit us and our dogs we would be more that happy to set up an appointment for you.

Everyone whom purchasing a pup from Goldenridge Kennels will receive a copy of Chapter 745 ‘’Sale of Dogs and Cats’’, which is from the State of Maine / Department of Agriculture.
　
The litter/all the puppies will be sold with AKC limited registration papers and with a spay and or neuter contract. Once I receive the spay and neuter Certificate from you or your veterinary then I’ll mail you your AKC paperwork. The litter/each pup will have a first distemper shot, general wormer and physical by the veterinarian.

To hold a puppy for purchase a $300.00 dollar non--refundable deposit is required for the
American Reds --$400.00 -- dollars non-refundable -for the blends -- $500.00 dollar non--refundable deposit for the Full Euro Crème Puppies.

The balance is due in cash only or Certified bank check payable to Roxanne Ellsworth
There is a 5% sales tax due on your puppy when you pick up your puppy.
when the buyer picks up puppy to take the pup home. If the puppy goes on an airplane there is also a fee for shipping/crate via air. The balance due on the puppy, airfare/crate cost to be paid by buyer 14 days prior to shipping of puppy via western union. The western union to be payable to Roxanne Ellsworth. If you elect to pay by check if your puppy is shipped by plane then I require check to be sent to me by the puppies 6 week of age.

Please note:
If a situation comes up for you or me or our female dog (for an example a false pregnancy) then your
deposit will not be refunded as mentioned above but will be applied to the next upcoming litter or another litter of your liking within two years from the date of your original deposit. If your puppy isn’t picked up by yourself or someone you’ve designated to pick up your puppy by the time your puppy is 9 weeks of age then: Twenty dollar per day fee will be applied for each day until your puppy is picked up for board/feed/care of your puppy which is due in the form of cash only when you pick up your puppy to take home.
